There seems to be two groups of Look up tables in the drop down menu, a top group and a bottom group. These groups of LUTs are seperated by a thin black line. I have noticed that the LUTs belonging to the bottom group, do not change the color of the image but instead, open up a new color bar named "Untitled". The LUTs in the top group behave normally ( the top group includes Fire, Grays, Spectrum etc; the bottom group includes 16_colors, 5_ramps, 6_shades etc.)

This has only been a problem since I recently upgraded FIJI. On another system where FIJI has not been upgraded there is not an issue with the LUTs. 

This problem also persists in different file formats, including .tiff; .jpg; and .asc (imported as a text image).

Indeed! This is caused by SCIFIO loading the lookup table and displaying it as an image. As a work-around, you can check off "Use SCIFIO" in Edit>Options>ImageJ2 to make it work again, while we are busy fixing the problem! (I actually have half of the fix already, but it is time to stop working for today).

I uploaded the fix, so you should be fine even with activating SCIFIO in the ImageJ2 options again.
